NEW AUTHORIZED PROVIDER AGREEMENT
 

Please sign and return as soon as possible.  This agreement will allow you to fully utilize the new program materials when they are released.

•  2011 AP Agreement
• New AP fee effective July 1, 2011
• Agreement allows AP's access to FREE online materials, multimedia presentations and the ability to library materials starting in April 2011
